What is Case Management?
Addiction affects every part of you: mind, body, and spirit. To find lasting recovery, you’ll need to work with a network of providers who specialize in each of these areas. Realizing this, we have made care coordination a top priority in our practice.
Acworth Outpatient Detox’s case management services are designed to make your life easier. Your dedicated case manager will become an expert on your needs. They’ll also serve as your advocate throughout the treatment process. As you recover, they will help you to address underlying health problems, find 12-Step resources, and guard against relapse.
Our case management services include the following.
Medication
In early recovery, you may benefit from withdrawal symptom management, as well as medication for co-occurring mental health conditions. Our medical staff can prescribe medication as needed.
Treatment Plan
Your treatment plan serves as a roadmap to recovery. It will include a list of supportive services that you will need at certain milestones in your journey to sobriety. Each client’s plan is completely individualized and based on their unique needs.
Medical Follow-Up Appointments
Often, addiction causes a host of health problems. Once you stop using substances, you will be in a better place to treat these conditions. Our case management team recommends nearby specialists and physicians to clients in need of medical support services.
Sober Living
If you need a high-accountability sober living environment, we can help. Acworth Outpatient Detox has partnered with a host of reputable sober living homes within the greater Acworth area.
